Step 1:

A) Navigate to the Superior Mobile Medics Website.

B) Click on the [Examiners] link icon located at the top of the page as shown.














Step 2:

A) After clicking on the Examiners icon in the previous step, you will be taken to the Orders Detail page.      

B) Input your Examiner ID and Password.

C) When finished, click on the [Log In To My Orders] button as shown.




Step 3:

A) Click on the order link you wish to generate a Zip Slip for. In this example, we will generate a Zip Slip for order number 1208243.  


Step 4:

A) After clicking the order link in Step 3, you will be taken to the Order Details page (note the highlighted CRL Zip Slip reminder located in the center of the page.)

B) Click on the [Generate CRL Zip Slip] button.

C) After a brief processing delay, the newly created Zip Slip will automatically display. Please note that you will need to have Adobe Acrobat, or Acrobat Reader installed to view and print the Zip Slip.

NOTE: if you do not have Adobe Acrobat Reader installed, you may download it for FREE from www.acrobat.com/reader.


 

 

 

 

 

 


Step 5:

1. Select the printer you wish to use to print out the newly created Zip Slip.

2. On the Acrobat print dialog, locate the “Page Handling” section, then select “Shrink to Printable Area” under Page Scaling.

3. Once you have set your printer output options, click the [OK] button to begin printing the Zip Slip.


 

 

Zip Slip Form Instructions

  • Zip Slip form must be legible including Notice Consent and Chain of Custody Statements at the bottom of the form. If form is illegible when it arrives at CRL the sample cannot be processed.
  • If any corrections need to be made to the preprinted fields then discard the Zip Slip form and use the consent form included in the CRL kit instead.
  • All non-preprinted fields are to be completed in the field by the examiner/applicant.
  • Remove EXTRA LABEL from consent in CRL kit and place on Zip Slip form where indicated.
  • Return the Zip Slip form to CRL with the specimen.
  • Discard the original consent form from the CRL kit when the Zip Slip form is used.
  • When using Zip Slip, the proposed insured will not receive a copy of the ZipSlip.

Before discarding the lab slip, please remove the TAMPER EVIDENT TAPE
at the bottom of the lab slip and place over collected sample.

If the applicant is applying for two carriers, write the second carrier name and home office location on one zip slip and send the completed and signed zip slip to CRL.

Important Do’s and Don’ts

01. Require physical delivery of consent to examiners for them to understand what the form should look like when printed.

02. CRL Kit consent barcode: Required to be applied to the center of the Zip Slip to link the tubes to the Zip Slip.

03. Zip Slip consent should be legible and sized as the sample provided during training.

04. The Zip Slip should not be in color ink, or on color paper.

05. Print the slip on one side only - Do not use light weight or card stock paper.

06. Note the size of Zip Slip is important for readability.

07. Comments related to the collection need to be communicated to the carrier are acceptable in the comment box

08. Omit comments such as:  FedEx tracking number, barcode of draw, examiner name, examiner office, any type of office stamps.

09. Avoid Faxing copies of the Zip Slip to CRL – always require the original Zip Slip.

10. Avoid sending copies of the Zip Slip to CRL – always require the original Zip Slip.

11. For multiple insurance companies – A single Zip Slip with a listing of multiple companies is preferred.
