How do I fill out a FedEx Airbill?
You should always have some of our preprinted FedEx Airbills. If not, please request
them as soon as possible and we will send them to you the same day. If you do not
have a pre printed FedEx Air bill please go to our website http://www.superiormobilemedics.com/
under generic forms and fill out exactly as shown.
“Do you rent /sell EKG machines, Centrifuges, Medical equipment, etc.?”
No, we do not rent /sell any medical equipment. A couple of resources with great
prices are EBay and Craigslist.
“When will I get my kits?”
Allow 5-7 business days for your supplies to arrive. Always order your supplies
at least a week in advance of your scheduled exam. You will receive an email confirmation
from us when we ship your kits. You can track your shipment through your email confirmation.
“How do you ship?”
Fedex and USPS Ground
“How many kits should I order?”
Please go online to our website http://www.superiormobilemedics.com You can check
to see what we have shipped you in the past and see what our records show that we
owe you in supplies.
“Do I need to be home to sign for my kits?”
No, only if you request a signature confirmation.
“What do you supply to the independent contractor?”
Kits(Blood, Urine, Saliva, DBS),Pre-Printed FedEx Airbills, FedEx Lab Bags, Butterfly
needles(23Gauge only), EKG supplies-Mounts, paper rolls, Square Z fold, Electrodes
